Responsibilities

POSITION SUMMARY:

Under the agencies policies and procedures and professional requirements of the role the registered dietician collaborates with providers and care team members in an integrated care team to identify and connect with patients in need of nutritional counselling. Meets with patients to identify educational needs and provide individual instruction utilizing resources based on the patients level of health literacy. The Registered Dietician participates in quality improvement activities and program evaluation procedures.Will attend clinical and operational team meetings as indicated to improve patient care and process. Will be knowledgeable related to population health management and engaging vulnerable populations. This position will report to the Director of Medical Services Management.

Duties and Responsibilities:

  • Assess patients health needs and current status. Establishes treatment plans and communicates with the patient regarding compliance with treatment.

  • Documentation is clear, concise and timely and meets of all internal and external requirements

  • Educates patients on chronic disease and the role of good nutrition in their treatment.

  • Develops resources and community contacts to meet the needs of patients at varying levels of health literacy.

  • Provides group education to people with similar needs to improve their health.

  • Works closely with providers and participates in huddles and team meetings

  • Established therapeutic relationships with a variety of patients for the purpose of establishing and reaching appropriate health goals.

  • Provides support and services at other treatment sites as needed.

  • Identifies potential barriers to quality care and suggest opportunities for improvement and reports these to the Director of Medical Services and Management.

  • Maintains appropriate knowledge, skill and certification for this position. Completes required education annually and provides documentation of these activities to her clinical supervisor.

  • Facilitates team building by providing support and guidance to other clinical/administrative staff when needed.

Qualifications

  • Education: Bachelors degree in nutrition and R.D. registration

  • Experience: A minimum of 2 years experience with prior chronic disease managment preferred

  • Knowledge, Skills and Abilities: Interpersonal skills which allow for professional and positive relationships with co-workers, patients, families and external customers. Good communication skills as evidenced by the trauma informed and language of caring approach. Effective writing, speaking and listening skills.
